Step 1 – Complete the relevant Insurance Election Form

Step 2 – Complete the ING Personal Statement if required
(for Personal Plan applicants; all requests in excess of 7 units of cover or $750,000; or any yes responses to the questions in the Insurance Election Form)

Step 3 – Send your form/s to us
Send your original signed and completed paperwork via post to:

Please note: you may be requested to provide further medical evidence or attend a medical examination if required.
